Job summary

Infusion Solutions, Inc. seeks a Patient Care Coordinator to support clerical and clinical tasks in the nursing department. You will help schedule SOC and office/home visits, triage calls, and ensure accurate charting with RN itineraries.

Qualifications

• High school graduate or GED.
• Associate’s degree or higher in healthcare administration, medical assisting, or a related field.
• Two or more years of healthcare coordination experience preferred.
• Experience with CareTend or similar systems preferred.
• Knowledge of insurance verification and healthcare billing workflows preferred.
• Excellent English communication.
• HIPAA confidentiality awareness.

Responsibilities

• Evaluates nurse staffing in the morning to determine coverage or rearrange schedules.
• Schedules SOC appointments with the Charge Nurse.
• Prepares SOC folders and conducts RN SOC Review after the first appointment.
• Schedules in-office and home visits considering patient environment.
• Covers CNA and Scheduler duties during absences.
• Answers phones, triages calls, and communicates clearly with patients and providers.
• Audits chart notes and RN itineraries; reports discrepancies to Clinical Nurse Manager.
• Completes patient discharges and maintains nursing logs in CareTend.
• Provides patient education on treatment and ongoing care.
• Maintains infection control policies and a clean work environment.
