Job summary

Infusion Solutions Inc in Washington state is seeking a Patient Care Coordinator (PCC) to support clerical and clinical tasks for the nursing department. You will assist with SOC scheduling, hospital discharges, and coordination with RN staff, pharmacists, and clinical liaisons.

Qualifications

• High school graduate or GED.
• Education across age groups from neonate to geriatric through coursework or experience.
• Ability to communicate effectively in English, both verbally and in writing.
• Preferred: Associate's degree or higher in healthcare administration, medical assisting, or related field.
• 2+ years of experience in healthcare coordination, infusion therapy, or home health settings.
• Experience with CareTend or similar scheduling systems.
• Familiarity with home infusion services, specialty pharmacy, or nursing workflows.
• Experience supporting multidisciplinary care teams.
• Knowledge of insurance verification, authorization processes, or healthcare billing workflows.

Responsibilities

• Evaluates nurse staffing in the morning to determine coverage needs and potential schedule rearrangements.
• Schedule SOC and in-hospital start-of-care appointments with the Charge Nurse.
• Prepare SOC folders and RN SOC Review after first appointment.
• Schedule in-office and home visits considering environmental conditions.
• Cover CNA and Scheduler duties during absences, including next-day scheduling and patient rooming.
• Triages incoming nursing calls and clears issues promptly.
• Review chart notes and itineraries; audit for discrepancies.
• Complete patient discharges daily and aggregate weekly visit counts.
• Audit RN itineraries against documentation and input visits into the nursing log.
• Interpret physician orders with RN scheduling.
• Educate clients/caregivers on treatment and ongoing care.
• Maintain confidentiality and infection control standards; use MS Office and CareTend.
• Other duties as assigned.
