Advanced Infusion Care in Garden Grove, CA is seeking a Patient Care Coordinator to organize nursing service schedules and provide administrative support. You will work closely with Intake, Pharmacy, Nursing and Home Health teams to coordinate referrals, visits and clinician schedules.

The role emphasizes timely communication, accuracy in scheduling, and maintaining patient privacy under HIPAA. This full-time position offers a comprehensive benefits package and a Monday–Friday schedule.

Competenze

• High School Diploma or GED required.
• At least 1 year in clinical support, admin, scheduling or customer service.
• Strong customer service and communication skills.
• Excellent organization and attention to detail.
• Ability to multitask and meet deadlines in a fast-paced setting.
• Proficiency with Microsoft Office and email apps.
• Experience navigating healthcare software systems.

• Coordinate scheduling for new patient referrals and ongoing nursing visits.
• Collaborate with Intake, Pharmacy and Nursing to ensure timely scheduling.
• Verify patients are ready for services and confirm visit dates.
• Communicate visit timelines and status updates to patients.

• Create and maintain clinician visit logs.
