Job summary

You will coordinate Start of Care appointments, in-office and home visits, and patient discharges, while triaging calls and auditing patient charts. Qualifications

• High school diploma or GED required.
• Education in age groups from neonate to geriatric via schooling, experience or training.
• Ability to communicate effectively in English, both written and spoken.

Responsibilities

• Evaluate nurse staffing in the morning to determine coverage needs.
• Schedule SOC and in-office/home visits.
• Prepare SOC folders and patient teaching materials.
• Triages incoming calls; review and audit chart notes daily.
• Discharge patients and generate weekly visit reports.
