Job summary

Cardinal Health is seeking a Patient Navigation Specialist for the cCARE Fresno clinic to coordinate patient needs and ensure access to resources. You will act as a central liaison among patients, physicians, and support services, resolving issues with diplomacy and professionalism.

Responsibilities include assessing patient care needs, facilitating referrals, collecting data on barriers, and contributing to staff and patient education.

Qualifications

• 0-2 years of experience preferred.
• Bachelor's degree in related field, or equivalent work experience, preferred.
• Direct patient care or Social Work experience highly desired.
• Ability to calmly and professionally resolve patient issues with diplomacy.

Responsibilities

• Assess patient care needs for new and ongoing patients.
• Coordinate with physicians and clinical offices to ensure access to resources.
• Serve as central contact for treatment patients and liaison between patients and providers.
• Collect and report data on barriers and intervene for optimal care.
• Assist patients with referrals to outside resources and care coordination.
• Adhere to departmental policies and quality improvement objectives.
• Collaborate with care teams to plan education for patients and staff.
• Design or participate in creating educational materials.
• Perform miscellaneous duties as assigned.
