Job summary

Cardinal Health Fresno is seeking a Patient Navigator to support practice operations, coordinating patient care and addressing provider and patient needs within a busy clinical setting.

The role emphasizes collaboration with physicians, nurses, and administrative staff to ensure access to resources and smooth referrals, while maintaining high patient satisfaction and adherence to policy and quality standards.

Qualifications

• 0-2 years of experience, related field preferred or equivalent work experience.
• Bachelor's degree in related field preferred, or equivalent experience.
• Direct patient care or Social Work experience highly desired.

Responsibilities

• Assess patient care needs and coordinate access to resources.
• Serve as essential link between patients and providers.
• Coordinate referrals to outside resources and care coordination as needed.
• Assist in creation of educational materials and staff education.
• Follow departmental policies and continuous quality improvement objectives.
• Perform miscellaneous duties as assigned.
