Job summary

APremium Healthcare Solution, LLC is seeking a compassionate Patient Navigation Assistant. You’ll guide patients through their healthcare journey, scheduling appointments, referrals, and follow-up coordination.

You will serve as the primary contact, addressing questions with empathy and professionalism, and collaborate with clinicians and administrative staff to ensure continuity of care. Experience with EHR, strong communication, and knowledge of community resources are valued; bilingual

Qualifications

• High school diploma or GED required; associate's or bachelor's in healthcare preferred.
• Experience in healthcare, patient services, or care coordination is advantageous.
• Familiarity with medical terminology, EHR, and healthcare workflows.
• Strong interpersonal and communication skills with a patient-first mindset.
• Bilingual candidates are a plus.

Responsibilities

• Guide patients through the healthcare system, including scheduling appointments, referrals, and follow-up care coordination.
• Serve as a primary point of contact for patients, addressing questions with empathy and professionalism.
• Assist patients in understanding treatment plans, discharge instructions, and available resources.
• Collaborate with clinical staff, physicians, and admin teams to ensure continuity of care.
• Maintain accurate patient records in the EHR system.
• Identify and help remove barriers to care, including insurance, transportation, and social determinants.
• Monitor patient progress and proactively follow up to ensure adherence to care plans.
