Job summary

UNLV Health is seeking an experienced Clinical Patient Navigator to guide patients through their outpatient care journey. You will coordinate appointments, referrals, and follow-up care, working closely with clinicians and staff to improve outcomes.

The role emphasizes helping patients navigate insurance, access resources, and overcome barriers to care, including language needs and transportation, while documenting progress and staying compliant with clinic policies.

Qualifications

• Minimum 3 years of experience in healthcare
• Medical Assistant Certification required (CMA/CCMA/NCMA/RMA)
• BLS certification required
• CMS-compliant patient navigator training (George Washington University) within 3 months of hire
• Bilingual Spanish preferred

Responsibilities

• Serve as the primary contact for patients with higher acuity level complaints and address questions about their healthcare journey.
• Ensure critical calls are handled efficiently and effectively.
• Gather essential information such as patients' symptoms to follow written standing orders.
• Collaborate with interdisciplinary teams to assist patients with needs.
• Coordinate appointments, referrals, and follow-up care for continuity across UNLV Health.
• Monitor patient progress and identify barriers to care.
• Assist patients with insurance and financial assistance navigation.
• Help patients overcome barriers to care (transportation, childcare, finances, supportive services).
• Connect patients with scheduling, tests, or referrals to specialists.
• Identify language barriers and arrange medical interpreters as needed.
