Job summary

Qualifications

• 0-2 years of experience, preferred
• Bachelor's degree in related field, or equivalent work experience, preferred
• Candidates with direct patient care or Social Work experience, highly desired
• Ability to calmly and professionally resolve patient issues with diplomacy

Responsibilities

• Assess patient care needs for new and ongoing patients.
• Coordinate with physicians and office staff to ensure access to resources.
• Serve as main link between patients and providers; central contact for treatment patients.
• Collect data on barriers and intervene for better patient care.
• Assist with referrals to outside resources and care coordination.
• Follow department policies and continuous quality improvement objectives.
• Collaborate on patient and staff education.
• Create or contribute to educational materials.
• Perform other duties as assigned.
