This entry-level role reports to the Director of Nursing and coordinates patient-centered care across referring physicians, hospital facilities and collaborating healthcare entities. The position emphasizes tracking orders, maintaining records, and guiding patients through the healthcare system to improve outcomes.

Qualifications

• Two years of directly related healthcare experience.
• Knowledge of medical terminology.
• Excellent written and verbal communication skills.
• Experience with Electronic Medical Records (EMR) systems.

Responsibilities

• Track and monitor provider orders for outpatient services including lab, radiology, diagnostic testing, and consults.
• Create and attach orders for outpatient services requested by specialists.
• Ensure medical records are available at scheduled appointments.
• Contact patients and/or specialists for missing reports.
• Update patient demographics as needed.
• Provide patients with copies of lost or misplaced orders.
• Facilitate follow-up appointments and patient portal engagement.
• Notify clinical staff of patient non-compliance.
• Support efforts to improve patient compliance and reduce gaps in care.
• Attend mandatory meetings and trainings.
• Other duties as assigned to support organizational goals.
