ChenMed LLC is seeking a Care Facilitator who provides exceptional customer service as the first patient contact. You will check patients in/out, schedule appointments and handle basic administrative tasks to support clinic operations.

The role requires a friendly, organized individual with healthcare exposure, English fluency, and willingness to travel locally up to 10% as needed. Evening/weekend shifts may be required.

Formación

• High school diploma or equivalent required.
• Graduation from a nationally accredited Medical Assistant program preferred.
• Minimum 1 year of work experience in a medical clinic or similar environment required.
• BLS for Healthcare Providers preferred.

Responsabilidades

• Greet, check-in and check-out patients and schedule appointments.
• Maintain lobby organization and cleanliness.
• Coordinate PCP and Specialists schedules, including ensuring top patients are scheduled every 13 days.
• Use computer systems to add patient data, print forms, schedule appointments, respond to emails and generate patient packets.
• Document patient information and insurance details, collect copays as needed.
• Place love calls and send birthday/greeting cards to patients.
• Answer phones and route messages to appropriate staff.
• Communicate clearly about ChenMed staff to patients and others.
• Verify patient insurance via phone/website.
• Assist with follow-up scheduling and in-house referrals.
• Escort patients to exam rooms and ensure comfort and wellbeing.
• Support PCP HEDIS needs and report hospitalizations as needed.
• Present surveys at checkout and escalate service issues.
• Participate in daily/weekly huddles with team.
• Coordinate transportation (Uber/Lyft) and medication deliveries.
