Job summary

Community Health Resources is seeking a Recovery Support Specialist for Pathways OTP in Putnam, CT. This part-time role supports clients in identifying recovery goals, provides coaching, and links them to behavioral health resources.

You will conduct assessments, help craft person-centered plans, and participate in crisis intervention as needed. The position requires lived recovery experience, 2+ years of relevant work, and certifications or eligibility within 6 months, plus a valid driver's

Qualifications

• Lived experience establishing and maintaining recovery from mental health or substance use condition(s) as required by position.
• 2+ years working with identified populations; RSS preferred; self identifies as in long-term recovery.
• Recovery Support Specialist Certification from Advocacy Unlimited or CCAR, or equivalent; valid driver's license.

Responsibilities

• Assist clients in identifying and advocating their own recovery goals.
• Collaborate with clients to create individualized and person-centered treatment plans.
• Provide crisis intervention or de-escalation as needed.
• Maintain documentation in compliance with agency and program standards.
• Transports clients as required by program.
