Job summary

Community Health Resources, Inc. (CHR) in Putnam, CT, seeks a Recovery Support Specialist (Part-time) for Pathways OTP. You will assist clients in identifying recovery goals, provide coaching, and collaborate with care teams to implement person-centered plans.

Requirements include lived recovery experience, 2+ years in related work, and relevant certifications or the plan to obtain them within 6 months; a valid driver's license is also required.

Qualifications

• Lived experience establishing and maintaining recovery from mental health or substance use condition(s).
• Must identify as a woman in recovery; 2+ years working with identified populations preferred.

Responsibilities

• Assists clients in identifying and advocating their own recovery goals.
• Utilizes motivational interventions and educates on recovery strategies.
• Conducts program assessments and screens as indicated.
• Collaborates to create individualized treatment plans.
• Provides crisis intervention and de-escalation as needed.
• Provides case management and links clients to supports and resources.

Education

Lived recovery experience

Recovery Support Specialist Certification or CCAR certification
