Job summary

Community Health Resources, Inc. (CHR) seeks a Recovery Support Specialist for the Mobile Pathways program on a per diem basis. The role is on-site in Putnam, CT with shifts Monday-Saturday 5:30AM-2PM, serving clients in MOUD and recovery support services.

Qualifications include lived recovery experience, 2+ years with relevant populations, and required certifications or must be obtained within 6 months. A valid driver’s license is required and CHR offers comprehensive benefits and professional

Qualifications

• Lived experience establishing and maintaining recovery from mental health or substance use condition.
• 2+ years working with identified populations; RSS preferred.
• Certification in Recovery Support or Recovery Coaching or equivalent required or must be obtained within 6 months of hire.
• Valid Driver’s License required.

Responsibilities

• Assists clients in identifying and advocating their own recovery goals.
• Utilizes motivational interventions to educate on recovery strategies.
• Conducts program-specific assessments and screens.
• Collaborates with clients to create individualized, person-centered treatment plans.
• Provides crisis intervention or de-escalation as needed.
• Maintains documentation in compliance with agency standards.
• Transports clients as required by the program.
