Job summary

Community Health Resources is seeking a Recovery Support Specialist I for the Mobile OTP program in Putnam, CT. This full-time role supports clients with recovery goals, assessments, and care planning, primarily serving MOUD needs from a mobile unit and the Putnam OTP base.

You will collaborate with treatment teams, provide coaching, and participate in crisis response as needed. Requires relevant certification and a valid driver’s license, with ongoing professional development offered.

Qualifications

• Two or more years of experience with the targeted populations.
• Self-identify as someone in long-term recovery from mental health or substance use.
• Valid driver’s license is required or must be obtained.

Responsibilities

• Assist clients in identifying and advocating their recovery goals.
• Use program models including motivational interventions to support recovery strategies.
• Conduct program assessments and screen clients as indicated.
• Collaborate with clients to develop individualized, person-centered treatment plans.
• Provide crisis intervention or de-escalation when needed.
• Conduct case management and link to supports and resources.
