Community Health Resources, Inc. seeks a Recovery Support Specialist I for the Mobile OTP in Putnam, CT. This on-site, full-time role supports MOUD services and recovery planning, working with clients in the field and in clinics.

Shifts typically Monday–Saturday 6:00 AM–2:00 PM, with strong emphasis on outreach and crisis support. The role requires lived recovery experience, 2+ years of relevant work, and certifications or commitment to obtain them within 6 months.

Qualifications

• Lived experience of recovery from mental health or substance use condition(s).
• 2+ years working with identified populations preferred.
• Certification required or to be obtained within 6 months of hire; valid DL required.

Responsibilities

• Assist clients in identifying and advocating their recovery goals.
• Use program models including motivational interventions and education.
• Conduct assessments and screens as indicated.
• Create person-centered treatment plans with clients.
• Provide skills teaching and crisis intervention as needed.
• Offer case management and coordinate with treatment teams.
• Link clients to community resources and follow-up care.
• Engage with clients to reduce health care disparities.
• Maintain documentation per agency standards.
