Job summary

Heartland Hospice seeks an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care when it’s most needed.

You will work with an interdisciplinary team, document clinical visits, and coordinate with daytime staff to maintain continuity of care. A current RN license and CPR certification are required, with at least 1 year of nursing experience in hospice or related fields.

Qualifications

• Current RN license in the requested employment state.
• 1+ years of nursing experience.
• Hospice heart: compassionate, calm, and dependable in urgent care situations.
• Excellent clinical judgment and ability to provide support in emotionally intense settings.
• Comfortable providing care in patients’ homes or other residences.
• Valid driver’s license, automobile insurance, and reliable transportation.
• Current CPR certification.

Responsibilities

• Respond to patient needs after normal business hours.
• Serve as liaison between patients, families, physicians, and the hospice team during shifts.
• Document visits and communications accurately.
• Participate in quality assessment and performance improvement programs.
• Work independently while coordinating with the daytime care team.
