Job summary

Heartland Hospice seeks an After Hours / On-Call Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care when it is needed most.

You will report to leadership, provide direct patient care, and coordinate with the interdisciplinary team to manage symptoms and update the plan of care. This role emphasizes independence and continuity of care in home settings.

Qualifications

• Current RN license in the state of employment.
• CPR certification and current credentials.
• Ability to provide compassionate care in high-stress, after-hours settings.

Responsibilities

• Provide direct patient care after normal business hours in patients' homes or facilities.
• Serve as primary liaison between patients, families, physicians, and hospice team.
• Document visits and clinical notes accurately in the EMR.
• Participate in quality assessment and performance improvement programs.
