Job summary

Heartland Hospice is seeking an After Hours/On-Call Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays in patients' homes. You will provide direct care, coordinate with physicians and the hospice team, and document outcomes to ensure quality and continuity of care.

You will work with an interdisciplinary group, including LPNs, CNAs, and support staff, to manage symptoms and support families in emotionally intense settings while adhering to the plan of care and

Qualifications

• Current RN license in the state of employment or eligibility to obtain
• 1+ years of nursing experience
• CPR certification and ability to work after-hours in patient homes

Responsibilities

• Provide direct patient care after normal business hours
• Serve as liaison between patients, families, physicians, and hospice team during shifts
• Document clinical visits and communications in the EMR
• Participate in quality assessment and performance improvement programs
• Assure continuity of care and coordination with daytime team during on-call periods
