Concierge Home Care is seeking a Home Health Occupational Therapist (OT) for Jacksonville, FL. This role offers a flexible schedule and autonomy, with 1:1 patient care in the home setting. The OT develops individualized treatment plans, conducts home safety assessments, and supports patients’ independence in activities of daily living.

Candidates should have a Florida OT license, CPR certification, and the ability to supervise COTAs. Transportation and a valid driver's license are required.

Your Role as a Home Health Occupational Therapist

Qualifications
