Job summary

Concierge Home Care is hiring Home Health Occupational Therapists (OTR) for Orange Park, FL. This role offers a flexible schedule, autonomy and pay-per-visit compensation with earnings around $95K+ annually.

You will conduct patient evaluations, develop and implement individualized treatment plans, improve functional mobility, and educate patients and caregivers. 1:1 in-home care with strong clinical mentorship and access to modern EMR tools supports your growth.

Qualifications

Active, unencumbered Florida OT license.
Valid driver's license and reliable transportation.
CPR certification.
Ability to supervise COTAs.

Responsibilities

Conduct patient evaluations in the home.
Develop individualized treatment plans based on physician orders.
Provide therapy to improve functional mobility and daily living skills.
Educate patients and caregivers on adaptive strategies and safety.
Collaborate with physicians and interdisciplinary teams.
Supervise and delegate care to COTAs when appropriate.

Your Role as a Home Health Occupational Therapist
