Concierge Home Care is hiring a Home Health Occupational Therapist (OT) to provide 1:1 in-home therapy across St Augustine and St Johns County. Earn an annual salary of $110,000+ with potential for top performers to reach $175K+ and quarterly bonuses.

Qualifications

Active Florida Occupational Therapist license.
CPR certification.
Valid driver's license and reliable transportation.
Ability to supervise COTAs.
Home health experience preferred, but not required.

Responsibilities

Evaluate patients and develop individualized home health treatment plans.
Perform home safety assessments and recommend modifications.
Provide therapy to improve ADLs and functional mobility.
Educate patients and caregivers on adaptive techniques and equipment.
