Job summary

Concierge Home Care in Jacksonville, FL is seeking a licensed Occupational Therapist to deliver skilled home health services in patients’ homes, helping them regain independence. You will perform evaluations, create individualized plans, supervise COTAs when appropriate, and document visits with AI-assisted tools; flexible schedules and no hospital rotations support work‑life balance.

• Active, unencumbered Florida Occupational Therapist license.
• Valid driver’s license and reliable transportation.
• CPR certification.
• Ability to supervise Certified Occupational Therapy Assistants (COTAs).
• Home health experience preferred but not required.

Responsibilities

• Conduct patient evaluations and home environment assessments.
• Develop individualized treatment plans based on physician orders.
• Provide therapy focused on improving functional mobility and daily living skills.
• Educate patients and caregivers on adaptive strategies and safety techniques.
• Collaborate with physicians and interdisciplinary care teams.
• Supervise COTAs when appropriate.
• Document visits efficiently using AI-assisted tools.

Your Role as a Home Health Occupational Therapist

As aHome Health Occupational Therapist, you will provide skilled therapy services in patients’ homes to help them regain independence, improve daily function, and safely perform activities of daily living.
