Concierge Home Care in Panama City, FL is seeking a Home Health Occupational Therapist (OT) to provide 1:1 care in patients' homes across Bay & Gulf Counties. The role offers a flexible schedule and autonomy with telemetric charting and user-friendly EMR tools to save time on documentation.

You will evaluate patients, create individualized plans, and coordinate with physicians and the care team to support ADLs, mobility, and independence while guiding COTAs and ensuring compliant documentation.

Qualifications

• Active Florida OT license
• CPR certification
• Valid driver’s license and reliable transportation
• Ability to supervise COTAs
• Home health experience preferred

Responsabilités

• Perform home safety assessments and recommend modifications
• Provide therapy to improve ADLs, functional mobility, and independence
• Educate patients and caregivers on adaptive techniques and equipment
• Collaborate with physicians and interdisciplinary teams

Your Role as a Home Health Occupational Therapist

Qualifications
