Job summary

Concierge Home Care in Fort Walton Beach, FL, is seeking a Home Health Occupational Therapist to deliver 1:1 in-home therapy across Okaloosa County. Flexible schedule and autonomy are emphasized, with no hospital rotations and mileage reimbursement.

You will evaluate patients, develop plans, perform home safety assessments, and educate patients and caregivers on ADLs and equipment. Collaboration with physicians and supervision of COTAs are expected, with documentation via WellSky/Kinnser EMR.

Qualifications

• Active, unencumbered Florida Occupational Therapist license required.
• Experience in home health preferred but not required.
• Ability to supervise COTAs and work independently.

Responsibilities

• Evaluate patients and create individualized treatment plans.
• Perform home safety assessments and recommend modifications.
• Provide therapy to improve ADLs, mobility, and independence.
• Educate patients and caregivers on adaptive techniques and equipment.
• Collaborate with physicians and interdisciplinary teams; supervise COTAs.
• Complete documentation using streamlined technology.

Your Role as a Home Health Occupational Therapist

Qualifications
