Concierge Home Care in Jacksonville, FL is seeking a Home Health Occupational Therapist (OTR) to deliver patient-centered care in the home with a flexible schedule and strong autonomy.

Responsibilities include conducting patient evaluations, creating and implementing individualized treatment plans, educating patients and caregivers on safety and adaptive strategies, and collaborating with physicians while supervising COTAs as appropriate.

Qualifications

• Active, unencumbered Florida Occupational Therapist license
• Valid driver's license, reliable transportation, and auto insurance
• CPR certification
• Ability to supervise COTAs
• Home health experience preferred, but not required
