Job summary

Concierge Home Care is seeking a Home Health Occupational Therapist (OTR) to provide 1:1 in-home evaluations and treatment in Nassau County, FL. The role offers a flexible schedule, autonomy, and pay-per-visit options, with full-time or PRN commitments.

You will develop treatment plans, educate patients and families, and collaborate with physicians and the care team. LIC/CPR and driving are required; home health experience preferred.

Qualifications

Active, unencumbered Florida Occupational Therapist license.
Valid driver’s license with reliable transportation.
CPR certification.
Ability to supervise COTAs.
Home health experience preferred, but not required.

Responsibilities

Conduct patient evaluations and home environment assessments.
Develop individualized treatment plans based on physician orders.
Provide therapy focused on improving functional mobility and daily living skills.
Educate patients and caregivers on adaptive strategies and safety techniques.
Collaborate with physicians and interdisciplinary care teams.
Supervise and delegate care to COTAs when appropriate.
