Job summary

Concord Hospital is seeking a Patient Care Coordinator to provide clerical and data management support for patient registration, referrals, and phone management. The PCC serves as the primary access point for patients and families, ensuring timely, efficient, and compassionate customer service.

Responsibilities include greeting patients, verifying insurance, managing appointments, and coordinating care across primary, specialty, and ancillary services.

Qualifications

• High School degree or GED equivalency required.
• Proven customer service experience.
• Knowledge of medical office operations including scheduling, registration, EMR, patient flow, billing, coding, referrals, authorizations, payer guidelines, compliance, credentials, privileges, regulatory agencies.

Responsibilities

• Greets and arrives patients; secures signatures for required documentation and follows CHMG policies.
• Obtains and verifies insurance and demographic information.
• Makes collection attempts while educating on resources available.
• Reconciles work lists and reports.
• Facilitates patient care between primary, specialty, and ancillary services.
• Welcomes and coordinates intake of patient information for new and existing patients, and maintains medical records.
• Processes and routes all calls and responds to inquiries.
• Schedules, confirms or reschedules patient appointments.
• Promotes Patient Connect enrollment.
• Distributes mail, faxes and electronic desktops.
• Receives, tracks and educates patients on form completion requests.
• Supports patient-centered medical home culture.
• Works independently and in a team on projects and events.
