Job summary

Concord Hospital is seeking a Patient Care Coordinator to provide clerical and data management support for patient registration, referrals, and call handling. This role acts as the primary access point for patients and families, ensuring timely, efficient, and compassionate customer service.

The role requires a high school diploma or GED and proven customer service experience, with knowledge of medical office operations and EMR systems.

Qualifications

• High School degree or GED required; must pass annual registration competency exam.
• Proven customer service experience.
• Knowledge of medical office operations, including scheduling, registration, EMR, billing, referrals, authorizations, payer guidelines, compliance, credentials, privileges, regulatory agencies.

Responsibilities

• Obtain all prior auths / precerts for the Cardiovascular Institute, including testing and procedures.
• Obtain and manage VA insurance referrals and PCP referrals; manage precert work queue.
• Process and route incoming calls and respond to inquiries; greet and sign in patients; secure required signatures.
• Perform collections education and coordinate resources; reconcile work lists and reports.
• Coordinate intake of patient information for new/existing patients; schedule/confirm/reschedule appointments; run Televox reports.
• Support a patient-centered medical home culture and collaborate on projects and events.
