Job summary

Provider Partners Health Plan is seeking a Nurse Practitioner to oversee coordinated care for PPHP members in designated skilled nursing facilities. You will collaborate with primary care physicians to develop and implement an individual plan of care.

You will serve as medical liaison to the interdisciplinary team, perform preventive care, diagnose acute and chronic conditions, order tests, prescribe medications, and guide transitions of care in accordance with state licensure and PPHP standards.

Qualifications

• Licensed NP with appropriate specialty for state practice.
• Knowledge of Medicare/Medicaid and PPHP credentialing requirements.
• Proficiency with electronic health records and data reporting.
• Strong interpersonal and organizational skills for elderly care.
• Experience in case management and long-term care settings preferred.

Responsibilities

• Provide autonomous, comprehensive health care within scope of practice.
• Collaborate with PCP to develop and implement care plans.
• Adhere to documentation standards and PPHP policies.
• Coordinate transitions of care with physicians, families, and care teams.
• Explain treatment plans and educate patients and families as needed.
