Job summary

Provider Partners is seeking an ISNP Nurse Practitioner to deliver coordinated and comprehensive care to PPHP members in designated skilled nursing facilities. You will work with the member’s PCP to implement a care plan, order tests, prescribe medications, and coordinate with specialists as needed.

You will serve as a clinical liaison to the interdisciplinary team, ensuring smooth transitions and high-quality preventive care for elderly patients, in compliance with state licensing and PPHP

Qualifications

• Licensed as an Acute, Adult/Gerontological or Family Nurse Practitioner in the state of practice.

Responsibilities

• Provides coordinated and comprehensive care to PPHP members in skilled nursing facilities.
• Collaborates with primary care physicians to create care plans.
• Orders and interprets diagnostic tests, prescribes medications, and refers to specialists as needed.
• Ensures documentation meets PPHP standards and coordinates transitions of care.
• Acts as a liaison to the care team and communicates effectively with patients, families and staff.
