Job summary

Provider Partners Health Plan is seeking an ISNP Nurse Practitioner to oversee coordinated care for PPHP members in designated skilled nursing facilities. You will work with the member’s primary care physician to deliver exceptional health care and serve as the medical liaison to the care team.
