Job summary

The Provider Partner Health Plan is seeking an experienced Nurse Practitioner to deliver coordinated care for ISNP members in skilled nursing facilities. Working with the member's PCP, you will act as the medical liaison and drive preventive care, diagnoses, tests, medications and care plans for short- and long-term goals.

Responsibilities include collaborating with the care team, documenting per PPHP standards, and maintaining autonomy to make clinical decisions while ensuring seamless

Qualifications

• Clinical competence and understanding of evidenced-based practice.
• Proficient with computers; export data, create reports, and relay results.
• Strong commitment to the care of the elderly and their families.
• Solid interpersonal and communication skills.
• Good organizational skills.
• Admirable teaching skills.
• Strong customer service orientation.
• Minimum of 3 years clinical care experience.
• SNF/LTC experience preferred.
• Case management experience preferred.
• Knowledge of Medicare/Medicaid.
• Detailed knowledge of I-SNP MOC and I-SNP population.
• Proficiency in nursing/medical databases and Microsoft programs.
• Positive and enthusiastic attitude.
• Independent thinking, autonomy and problem solving.

Responsibilities

• Demonstrates autonomy and proficiency in comprehensive health assessments.
• Collaborates with the member’s PCP to create a preventive care plan.
• Adheres to PPHP documentation and communication standards.
• Observes the scope of practice in the licensed state.
• Communicates with physicians and family during transitions of care.
• Facilitates a medically appropriate plan of care to avoid duplication of testing and fragmentation.
