Job summary

Provider Partners Health Plan is seeking an ISNP Nurse Practitioner to deliver coordinated, comprehensive care for PPHP members in skilled nursing facilities. The NP collaborates with the member’s PCP to provide preventive care, diagnose illnesses, order tests, prescribe medications, and plan short- and long-term goals.

The role acts as a clinical liaison to the interdisciplinary team, ensuring seamless service delivery and adherence to the I-SNP MOC.

Qualifications

• Solid clinical competence and evidence-based practice.
• Proficient with computers; able to export data, create reports, and share results.
• Strong commitment to elderly care and family needs.
• Excellent interpersonal and communication skills.
• Good organizational abilities and teaching aptitude.
• Customer service orientation is essential.
• Minimum of 3 years clinical care experience.
• SNF/LTC experience and case management preferred.
• Familiarity with Medicare/Medicaid and I-SNP policies.

Responsibilities

• Demonstrates autonomy in comprehensive health assessments.
• Collaborates with PCP to create prevention-focused care plans.
• Adheres to PPHP documentation standards and care model.
• Works within state scope of practice.
