Job summary

Provider Partners is seeking an ISNP Nurse Practitioner to coordinate comprehensive care for PPHP members in designated skilled nursing facilities. You will work with the member's PCP to deliver exceptional health care and serve as a clinical liaison to the care team.

You will provide preventive care, diagnose acute and chronic conditions, order tests, prescribe medications, and establish short- and long-term goals. Experience in elderly care and case management is valued.

Qualifications

• Case management experience preferred.
• Collaborate with primary care physician to create care plans.
• Proficient documentation per PPHP standards.
• Observes state scope of practice.
• Strong communication with care team and families.

Responsibilities

• Provides autonomous, comprehensive health care in SNF settings.
• Collaborates with PCP to develop short/long-term care plans.
• Ensures documentation meets Provider Partners Health Plan standards.
• Communicates with physicians and families during transitions of care.
• Delivers preventive care, diagnoses, orders tests, prescribes medications.
