Job summary

Provider Partners Health Plan (PPHP) seeks an Institutional Special Needs Plan (ISNP) Nurse Practitioner to deliver coordinated care at designated skilled nursing facilities. You’ll collaborate with the resident’s PCP, provide preventive care, diagnose conditions, order tests, prescribe medications, and set short- and long-term goals.

Qualifications

• Currently licensed as an Acute, Adult/Gerontological or Family Nurse Practitioner in the state of practice.
• Meets Provider Partners Health Plan credentialing requirements.
• Holds or is eligible for DEA licensure and CDS licensure (if required in the state of practice).
• Basic Cardiac Life Support (BCLS) certification.
• Current unrestricted RN license for the state practicing.
• Valid driver license and car insurance with reliable transportation.

Responsibilities

• Demonstrates autonomy and proficiency in comprehensive health assessments.
• Collaborates with the PCP to create a prevention-focused care plan.
• Adheres to PPHP documentation and communication standards.
• Observe scope of practice per state license.
• Communicate with physicians and families during transitions of care.
• Coordinate with care team to avoid duplications and fragmentation of care.
