Job summary

Provider Partners Health Plan is seeking an ISNP Nurse Practitioner to deliver coordinated and comprehensive care for PPHP members in skilled nursing facilities. You will collaborate with the member’s PCP, acting as the medical liaison to the care team, and provide preventive care, diagnosis, and care planning with short- and long-term goals.

The role requires independent thinking, strong clinical skills, and experience with Medicare/Medicaid guidelines within the ISNP framework.

Qualifications

• Demonstrates autonomy in comprehensive health assessments.
• Collaborates with the PCP to create a prevention-focused care plan.
• Adheres to PPHP documentation standards and communication protocols.
• Observes state-licensure scope of practice.
• Communicates with physicians and families during transitions of care.
• Facilitates an effective, non-duplicative plan of care.

Responsibilities

• Perform comprehensive health assessments independently.
• Coordinate with PCP to develop preventive, early-detection, and timely intervention plans.
• Follow PPHP documentation standards and communication guidelines.
• Respect state scope of practice wherever licensed.
• Communicate with care team and families during transitions of care.
• Ensure coordinated, evidence-based care without redundant testing.
