Job summary
Provider Partners Health Plan seeks an experienced Nurse Practitioner to coordinate and deliver comprehensive care for PPHP members in designated Skilled Nursing Facilities. The NP collaborates with the member’s PCP, serves as medical liaison to the care team, and supports preventive care, diagnoses, orders tests, prescribes medications, and plans short- and long-term goals.

Qualifications
- Active NP license in the state of practice.
- RN license is current and unencumbered.
- DEA/CDS licensure as required by state.
- BLS certification is maintained.
Responsibilities
- Provide autonomous comprehensive health assessments.
- Collaborate with PCP to develop prevention-focused care plans.
- Adhere to PPHP documentation and care standards.
- Work within state scope of practice and transitions of care.
- Communicate with physicians, families, and care team effectively.
- Coordinate with interdisciplinary staff to avoid care fragmentation.
