Job summary

Provider Partners Health Plan is seeking an Institutional SNP Nurse Practitioner to coordinate and deliver comprehensive care for PPHP members in skilled nursing facilities. You will work with the PCP, serve as a medical liaison to the care team, and establish short- and long-term care goals for patients and families.

You will assess, diagnose, and manage acute and chronic conditions, prescribe medications, and refer to specialists as appropriate, while ensuring seamless, high-quality service

Qualifications

• Must hold valid Nurse Practitioner license in the practicing state.
• Meets PPHP credentialing requirements.
• DEA licensure/ CDS licensure (if required).
• Current unrestricted RN license in practicing state.
• Basic Life Support certification.

Responsibilities

• Conduct autonomous comprehensive health assessments.
• Collaborate with PCP to develop prevention-focused care plans.
• Adhere to PPHP documentation and communication standards.
• Remain within scope of practice in the practice state.
• Communicate with physicians and families during transitions of care.
• Coordinate with care team to prevent unnecessary testing and fragmentation of care.
