Job summary

Provider Partners Health Plan (PPHP) seeks an experienced Nurse Practitioner to lead clinical care for institutional special needs plan members in skilled nursing facilities. This role partners with primary care physicians to deliver preventive and episodic care, order tests, prescribe medications, and establish short- and long-term goals.

Qualifications

• Licensed Nurse Practitioner in state of practice (Acute, Adult/Gerontological, or Family).
• Current DEA and CDS licensure eligibility (where required).
• Basic Cardiac Life Support certification (BCLS).
• Current unrestricted RN license in practicing state.
• Experience with Medicare/Medicaid and I-SNP MOC knowledge.

Responsibilities

• Conduct comprehensive health assessments with autonomy.
• Collaborate with PCP to create prevention-focused care plans.
• Adhere to PPHP documentation and communication standards.
• Follow state practice scope and regulations.
• Communicate with physicians and families during transitions of care.
• Coordinate care with the interdisciplinary team to avoid duplications.
