Job summary

Matrix Medical Network delivers comprehensive health assessments in patients’ homes across the nation. As an NP provider, you will review a member’s health, medical history, medication adherence, and risks during visits that can last up to one hour, enabling targeted care improvements and gap closure.

You’ll administer testing as needed, collaborate with physicians and care teams, and provide care at members’ residences or SNFs.

Qualifications

• Master’s Degree or commensurate experience and satisfactory NP licensure.
• Current NP licensure in state of practice with prescriptive authority as needed.
• Board certification in Adult/Adult Gerontology/Family/ Emergency/ Acute Care.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license for the state of residence.
• Automobile insurance coverage for state of residence; see official coverage details.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other professionals.
• Coordinate with PCP on patient education and follow-up.
