Matrix Medical Network invites experienced NPs to join our national in-home health assessment program. Providers assess health history, current status, and medication adherence at the member’s residence, including SNFs, to identify care gaps and improve outcomes.

Formación

• Master’s degree or commensurate experience.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• AANP/AACN/ANCC board certification in suitable NP specialty.
• BLS/ACLS/CPR certification; 1 year NP experience preferred, new grads welcome.
• Valid state driver's license and auto insurance coverage.
• Strong computer skills and familiarity with health-record software.

Responsabilidades

• Conduct adult/geriatric assessments including health history, diagnoses, and care planning.
• Administer and order point-of-care and lab tests as needed.
• Collaborate with PCPs, case managers, social workers, and caregivers.
• Provide services at members’ homes or SNFs.
• Document findings clearly following Matrix guidelines.

Conocimientos

NP licensure in state

Board certification: Adult/Gero/Family

BLS/ACLS/CPR certification

Excellent verbal and written comms

Strong computer skills
