Matrix Medical Network is seeking Nurse Practitioners to perform comprehensive in-home health assessments. Each visit can last up to one hour, reviewing medical history, medications, and social factors to close care gaps.

Providers will work with physicians and care teams to deliver high-quality, patient-centered care in patients’ homes or SNFs, with a flexible schedule and opportunities for full-time roles.

Competenze

• Must have NP licensure in the practice state and relevant board certification.
• Master’s degree or equivalent experience required for skilled practice.
• Strong computer skills and familiarity with medical record software are expected.

Mansioni

• Conduct adult/geriatric health assessments during home visits and document findings per Matrix guidelines.
• Administer point-of-care and lab-based tests as needed.
• Collaborate with physicians, case managers, social workers and caregivers to coordinate care.
• Educate patients and follow up with PCPs for continued care and coordination.

Formazione

Master’s degree or commensurate experience

Current NP licensure in state of practice

Board certification (AANP/AACN/ANCC)
