Matrix Medical Network delivers comprehensive in-home health assessments to older adults and at‑risk individuals, visiting homes or SNFs with flexible scheduling and a focus on improving health outcomes.

As a Nurse Practitioner, you review medical history, medications, and risks, perform assessments, order tests, and coordinate care with physicians and care teams, while documenting according to Matrix guidelines.

Competenze

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice to include prescription authority.
• Board certification in relevant NP specialties.
• CPR/BLS/ACLS certification.
• 1 year NP experience preferred; new grads encouraged.
• Valid state driver’s license and auto insurance.
• Strong computer skills and familiarity with health records software.
• Excellent verbal and written communication skills.

Mansioni

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, family members, caregivers and other personnel as appropriate.
• Coordinate with Primary Care Physician on patient education and follow up as necessary.
• Provide services at the member’s residence which can be in their homes or Skilled Nursing Facilities.
