Matrix Medical Network provides in-home health assessments led by clinicians. This full-time NP role conducts geriatric assessments at patients’ homes or SNFs, reviews medical histories, medications, and social factors, and collaborates with PCPs to close care gaps.

You will perform exams, order tests, and document findings per Matrix guidelines, ensuring high-quality patient care and follow-up.

Qualifications

• Master’s Degree in NP and licensure to practice as an NP.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in one of listed specialties.
• BLS/ACLS certification current.
• Minimum 1 year NP experience preferred; new grads encouraged.
• Willing to travel up to 15%, including overnight.
• Valid driver's license and auto insurance availability.

Responsabilités

• Perform adult/geriatric assessments including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols.
• Order and administer point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and ancillary medical personnel as appropriate.
• Coordinate with PCP on patient education and follow up as necessary.
• Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities.
