Job summary

Matrix Medical Network is recruiting NP providers to perform comprehensive in‑home health assessments across the United States. During visits up to an hour, you review health history, medications, and risk factors to gain insight into overall well‑being.

You’ll conduct adult/geriatric assessments, order tests, coordinate with PCPs and care teams, and document findings per Matrix guidelines. Services occur at members’ homes or SNFs with flexible scheduling and a $1,500 sign‑on bonus.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority if needed.
• Board certification in Adult, Gerontology or Family across primary care specialties.
• BLS, ACLS or CPR certification active.
• 1 year NP experience preferred; new grads welcome to apply.
• Valid state driver’s license and auto insurance as required.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education and referrals.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with PCPs, case managers, social workers and caregivers.
• Document assessments clearly according to Matrix guidelines and protocols.
• Provide services at members' homes or Skilled Nursing Facilities.

NP licensure

Board certification (AANP/AACN/ANCC)

Strong computer skills / EHR software

Excellent verbal and written skills

BLS/ACLS/CPR certification

1 year NP experience preferred

Education
