Job summary

Matrix Medical Network focuses on in-home health assessments across the nation, seeking a Nurse Practitioner to perform comprehensive adult/geriatric assessments in patients' homes or Skilled Nursing Facilities. You will review medical history, medication adherence, and risk factors to guide care decisions.

Collaborating with physicians, case managers, social workers, and caregivers, you will support care coordination, order point-of-care testing, and document findings per Matrix guidelines,

Qualifications

• Master’s Degree in nursing or commensurate NP experience.
• Current NP license in state of practice with prescriptive authority.
• Board certification: AANP, AACN, or ANCC in relevant specialty.
• Active BLS/ACLS or CPR certification.
• Willingness to travel up to 80–90%, including overnights.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, social workers, and family members.
• Provide services at the member's home or Skilled Nursing Facilities.
