Job summary

Matrix Medical Network seeks Nurse Practitioners to perform Comprehensive Health Assessments in members' homes, reviewing health history, medication adherence, and social factors to improve outcomes. You will coordinate with PCPs and care teams while delivering high-quality, individualized assessments.

Role emphasizes in-home visits, flexible scheduling, and collaboration with physicians, case managers, and staff in diverse settings, including SNFs.

Qualifications

• Must hold NP licensure in the practicing state.
• AANP, AACN, or ANCC board certification in relevant NP specialty.
• Current BLS/ACLS or CPR certification is required.
• Valid driver's license and auto insurance for the state of residence.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric health assessments including history, diagnosis, treatment, and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Provide in-home services at the member's residence or in SNFs.
• Coordinate patient education and follow-up with PCP as needed.
