Job summary

Matrix Medical Network delivers in-home comprehensive health assessments across the nation. Providers review a member’s health, history, medication adherence, and social factors during visits that may last up to an hour.

Our clinicians perform geriatric assessments, order testing, and coordinate with physicians and care teams to close care gaps, all while enjoying flexible scheduling and opportunities for professional growth.

Qualifications

• NP licensure in state of practice required.
• Board certification in relevant NP specialty.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license.
• Auto insurance coverage meeting state requirements.

Responsibilities

• Conduct adult/geriatric assessments including medical history and treatment plans.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, social workers, and family.
• Provide in-home or facility-based assessments at members’ residences.
• Document findings clearly and follow Matrix guidelines and protocols.

Skills

NP licensure in state of practice

Board certification (ANCC/AANP/AACN)

BLS/ACLS certification

1 year NP experience preferred

Valid driver’s license

Auto insurance coverage

EMR/health-record-software knowledge

Excellent communication skills

Education

Master’s Degree in NP (or commensurate experience)
