Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments, reviewing medical history, medications, and social factors to improve health outcomes. You will work with physicians, case managers, and caregivers to close care gaps through thorough documentation and patient education.

Visits occur in members’ homes or SNFs, with schedules based on your availability. Competitive pay, mileage reimbursement, and continuing education support are provided to help

Competenze

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice including prescriptive authority.
• Board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred, new grads encouraged to apply!
• Valid state driver’s license for the state of residence.
• Automobile insurance coverage requirements for the state of residence (or ability to obtain).
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.

Mansioni

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers as appropriate.
• Coordinate with PCP on education and follow-up.
• Provide services at the member’s residence, including homes or Skilled Nursing Facilities.

Formazione
